How Can I Obtain a Duplicate Czech Death Certificate?

A duplicate death certificate from the Czech Republic ("Úmrtní list") can only be obtained within the country. More specifically, it must be requested from the local civil registry office where the death was originally registered. The Czech Republic does not have any offices abroad that can issue this document.

Although officially called a duplicate, the document is identical to the original death certificate - the only difference is the current date of issue. The certificate itself does not indicate that it is a copy.

The most important point is determining who has the right to request this document. Immediate family members of the deceased (children, spouses, or grandchildren) can apply directly. Third parties may also obtain the certificate, but only with a certified power of attorney from the deceased's relatives (we can assist with arranging this if needed). Many civil registry offices require documents proving the applicant's relationship to the deceased. For example, children requesting a parent's death certificate should provide a certified copy of their birth certificate. Although some Czech civil registry offices may accept matching names alone, we strongly recommend preparing the proper documents to avoid complications.

How and Where Can I Get an Apostille for a Czech Death Certificate?

An apostille for a Czech death certificate can only be obtained within the Czech Republic, as the country has no offices abroad that provide this service.

The apostille process for either the original or duplicate death certificate can be handled by any third party without a power of attorney from the deceased's relatives (a power of attorney is only needed when obtaining a duplicate certificate). The apostille is issued by the Czech Ministry of Foreign Affairs. However, before applying to the Ministry, the death certificate must first be certified with a special verification stamp from the relevant regional or municipal administration, in addition to certification by the civil registry office. Without this step, the Ministry will not issue the apostille.
